The context of a form controls where a form can be used in the system.

Answer Types
When adding an Entry or Question to a form in Form Builder, you must select an Answer Type. The Answer Type you select can impact your reporting capabilities, thus it is important to select an Answer Type that will you allow to collect and view data your institution needs. Answer Type options in Form Builder are listed below, along with a description and example.
Note(s): Answer Type options that can be aggregated in a report are denoted by an asterisk (*).

Date | Form recipients can bring up an interactive calendar from which they select the appropriate date. |
Date/Time | Form recipients can bring up an interactive calendar from which they select the appropriate date. They can also select the hour, minute, and AM/PM. |
Single Line of Text | Inserts a single line of text into which users can enter a few words (up to 255 characters). |
Large & Small Text Areas | Inserts a text box into the form into which users can enter their response. Both the Small text area and Large text area Answer Types have the same character limits. The only difference between the two is the amount of physical space occupied on the form. |
Decimal Number (allows numerals and decimals) | The question asked requires entry of a number that may be in decimal form (e.g., 2.6, 1.5). If the answer does not require decimals, you can select Whole Number (allows only numerals). |
*Whole Number (allows only numerals) | The question asked requires entry of a whole number (a number that will NOT be in decimal form). If the answer requires decimals, you can select Decimal Number (allows numerals and decimals). |
*Likert Scale | Inserts a series of questions with a fixed set of answers into the form. To use this type of answer type, enter the possible answers (e.g., Agree, Neutral, Disagree) and point values for each possible answer. Add questions or add headings with instructions. |
*Multiple Choice/Drop-down Menu | Inserts a drop-down menu into the form from which users can select one correct answer. |
*Multiple Choice/Radio Buttons | Enables users to select one correct answer by filling in a radio button. |
*Multiple Choice/Checkboxes (multiple correct answers) | Enables form users to select more than one answer for an entry or question by checking a box next to each answer selection. |
*Table | Inserts a table into the form. Row entries will be completed by the recipients. |
*Rubric | Select to build a rubric within the form. Rubrics have several configurable options including number of criteria, number of performance ratings, point and range values, criterion description per performance rating, and comment per criterion. Can choose to remove NA on rubric criteria. |
*Scoring Rubric | Select to build a rubric that accommodates points and weights, including decimal numbers. Only available when form context is set to Field Experience Forms or Assessment Tools. |
Heading | Inserts a line of text with no answer. You can use a Heading to create sections, add additional instructions, or help organize your form. |
File | Allows users to upload a file from their computer into the form. Single file upload and multiple file upload options are available. |
User Attribute | Auto-populates an attribute of the form recipient into the form. The attribute auto-populates when the form is received by the recipient. User attribute allows you to view information about a user while minimizing user entry error. Available attributes include:
