Attaching Forms to DRF Templates

You can select a Custom Form (designed in the Form & Survey Builder) for Authors to complete as part of a requirement in the DRF. Their completed form can then be submitted to Reviewers and/or Evaluators.

To begin, click TS Coordinator.

Click the DRF Template Builder link.

Click the Edit button that corresponds to the DRF Template to which you wish to attach a form.

Click on the requirement where the form should be attached in the structure display in the left panel of the screen. The right panel refreshes to display that requirement.

Select the Custom Form check box for this requirement.

You will be navigated to an additional page where you can select an existing form from a pull-down menu or navigate to the Form Builder to build a new form.

Attach a Form as an Evaluation Method

Using a form as an evaluation method enables you to evaluate an area of a DRF against criteria defined in the form designed by you or someone from your organization. Forms are a flexible medium, enabling assessment criteria to be presented in a variety of ways including ratings scales, multiple selection checkboxes, and single response radio buttons. By including questions to be answered by free-form text of varying length, you can extend the scope of evaluation to provide focused commentary comments.

Click Tab 3 – Evaluation.

Click Assign Evaluation Method for the appropriate requirement.

Under Advanced Evaluation Methods, click the Select button that corresponds to Custom Evaluation Form.

Make selections for the following:

  • Choose a form you created, a form created by another individual in your learning community or click Go To Form Builder to create a new form.
  • Select the appropriate final scoring method.
  • Choose the form visibility. (Should the form be visible to the student in his/her evaluation report?)
  • (Optional) Add notes for the evaluators.

Click Apply Changes when finished.
