Add a Form Element

Taskstream forms are made of a series of elements that prompt respondents to answer questions or supply information. Whether you are creating a new form or editing an existing one, these elements are added from the Build Form screen.

The Build Form page also enables you to manage (including rearrange) the elements that you've already added.

To add a form element

From the Build Form page, click Insert New Form Element. You are navigated to the Add FormElement page.
Select the type of element you want to add from the Create new element pull-down list.

 You can also opt to import elements (queries and their responses) from your otherforms or from forms that have been shared by other TS Coordinators. Any form can include both new and imported elements.

(Optional) To quit your action, click Cancel.
Click Continue.
You are navigated to another screen, to establish the element you selected:

Section Header or Subsection Header. : dividers that organize form elements into meaningful sections

Open Ended (text/comment area): present a query that the responder will answer by entering text.

Open Ended - 2 column format (text/comment area): present a query that the responder will answer by entering two columns of text (for example, to compare two concepts).

Choice – one answer (radio buttons or pull-down).present a query with a set of response options from which the responder will select a single answer

Choice – multiple answers (check boxes).present a query with a set of response options from which the responder may select multiple answers

Rating Scale (single or multiple criteria can be rated): present a query with a scaled range of values from which the responder must choose.

Date: present a query that the responder will answer by entering a date.

Number: present a query that the responder will answer by entering a number.

When the element is saved, you are returned to the Build Form page to continue work on thisform.
When you have completed your work on this form, click the Preferences button to set sharing preferences.
To exit the Build Form page,

EITHER click the Form & Survey Builder breadcrumb to return to the home page for this tool,
OR click one of the links at the top of your page to navigate to another area of Taskstream.

Short Titles View

As you add elements to a form, you have the opportunity to enter a short title and/or descriptive text (such as directions) for each of them.

By default, the Build Form page displays the element title and (if any) directions. When working on a longform, you may want to streamline the appearance of the Build Form page by checking the Show Short Titles Only checkbox. This resets the page to display only the short title of each element.

To return to the expanded view, deselect the checkbox.

This option has no impact on the respondent view of the form. Those who are asked to fill in your formwill see the long title and descriptive text (if any) of each element.

To see what the respondent will see, click the Preview button.
