My community college is working on revising our current set of questions for students. Would anyone like to share the questions they use for course evaluations? Or do you have guiding goals, principles or procedures that help you design your questions? We'd love to see examples of any of that. And for the sake of sharing, I'll attach our current (and much in need of revision) question set to this post. Thank you!
We have information about our course evaluations and the questions we use on our website but these are the 10 standard questions we include on all of our course evaluations here at Stanford:
How well did you achieve the learning goals of this course?
Extremely well, Very Well, Slightly Well, Not well at all
About what percent of the class meetings (including discussions) did you attend in person?
Whole number percentage between 0 and 100
About what percent of the class meetings did you attend online?
Whole number percentage between 0 and 100
How much did you learn from this course?
A great deal, A lot, A moderate amount, A little, Nothing
Overall, how would you describe the quality of the instruction in this course?
Excellent, Good, Fair, Poor, Very Poor
What skills or knowledge did you learn or improve?
Open-ended
How many hours per week on average did you spend on this course (including class meetings)?
Whole number hours
How organized was this course?
Extremely organized, Very organized, Moderately organized, Slightly organized, Not organized at all
What would you like to say about this course to a student who is considering taking it in the future?

We have been using this set of questions since Fall 2015 and if you're interested, you can read the Course Evaluation Committee report that drove the adoption of this form. We also have a separate Section form to provide TAs with direct feedback and there is information about that form on this Section Feedback page.
We are also in the middle of piloting new questions for our campuses. We have 3 campuses all with different questions. This spring we piloted a set of 7 quantitative and 2 qualitative questions. These questions will be our core university questions. Campus/College/Departments will have the opportunity to add 1 or 2 questions of their own, leaving the bulk of the questions to the faculty to add for their own course management. You can find our survey instruments here: Survey Instruments (the pilot survey instrument will be posted in the next day or two). A book that has been extremely helpful in revising our instrument and the University's Faculty T&P process is Developing a Comprehensive Faculty Evaluation System by Raoul Arreola. I also have a list of other universities that came up in my research that I have also referenced. Reach out if you would like those. I will also be presenting this journey at Engage if you are able to attend.
